Here is a description of our online forms and when they are used:

 Client History Form

This form is used for gathering Animal behavior, performance and well-being history, and should be completed and submitted before your Holistic Animal Consultation, and other times, as requested by Kathy.

 Well-being Consultation Form

This form is used for helping to determine the focus of intuitive consultations, spa sessions and healing sessions for you or your animal.

 Client Agreement Form

This form is already incorporated into several other forms and the shopping cart, so it will be rarely used stand-alone. It outlines the scope of services, and Kathy may send you here if you have not accessed this form through other means.

Make a Payment Form

If you wish to make a secure online payment via credit card or PayPal for something that is not in the shopping cart, you may use this form.

Request an Appointment Form

This is an online form that you may use to request an appointment for many of the services offered at Dancing Hearts. Some of these services may also be purchased through the shopping cart.
